A Hospital Discharge Planning Checklist

During a stay in a hospital or care center, a doctor and the staff will work to plan for the patient's discharge. The patient and their caregiver(s) - family member or friend who may be helping - are important members of the planning team. Below is a checklist of important things the patient and the caregiver(s) should know to prepare for discharge. Use this checklist early and often during the stay. Talk to doctors and the staff (which could be a discharge planner, social worker, or nurse) about the items on the checklist.
